The link below will take you to our secure documentation system, IntakeQ, where you can enter your child’s information, upload copies of your insurance card, and share any concerns you have for your child. If you are unsure which services may be the best fit or have any questions that are not answered here, please contact our administrative team at 773.332.9439 or email us at evaluations@playworkschicago.com.
Our billing team will contact your insurance provider to verify your therapy-specific coverage. We will then email you an estimate of your benefits before your family’s first visit.
The PlayWorks Therapy Intake Coordinator will email you to confirm that we have received your intake form, review the recommended services based on the concerns you shared, and provide details about scheduling an initial evaluation. Once your child’s evaluation date and time are confirmed, you will receive a second email from IntakeQ with a link to complete the full intake paperwork packet and case history prior to the evaluation.
